Support Worker possible job share for the right candidates !!

As a successful candidate you will have a proven track record within this sector.

PURPOSE OF JOB:
To provide support and guidance for vulnerable Young People aged 16 – 18.

To support Young People who may have intensive or complex needs which may include drug and or illegal substance mis-use, through education and/or training, providing information, support and guidance in preparation of adulthood and independent living.

Provide targeted interventions with young people engaging in/or at risk of engaging in anti-social behaviour.

Support workers will develop effective working relationships with residents and team members. They will maintain the safety and welfare of the residents, planning, monitoring and developing skill requirements, providing opportunities for young people to engage in their personal development.

Support workers to take ownership of developing communication and practical skills enabling residents to make decisions in how best to progress in their future goals.

Working closely with other professional bodies from a range of Local Authorities, external Support Agencies to support Young People in their endeavour to become self-sufficient within the community by promoting the development of evidence-based practice/workshop sessions.

Engage with young people (16 -18) and understand/listen to their views and opinions in order to identify and address their individual needs, which effect their personal and social development, career aspirations, education and employment prospects.

Collect and manage information to facilitate the process of meeting the needs of your Young People through preventative measures and key interventions.

Commitment to planning and assessing the improvement of the support that is being delivered.

Utilise and support education and training providers, employers and communities in meeting the needs of our Young People.

Gain commitment to opportunities by working with Young People, Local Authorities, Social Workers and supporting External Agencies to engage young people in positive activities, both formal and informal.

To manage a personal caseload of Young People providing sustained support to individuals regarding actual or perceived obstacles and barriers affecting their successful move to adulthood. This may include advocating on behalf of Young People, facilitation of group work or other one-to-one interventions.

To liaise with other support agencies as necessary. To identify and contact external agencies who may provide a suitable service.

Effective communication and information sharing between professionals and Young People including common understanding of the key terms and objectives for positive outcomes.

To support professionals within the Young Person’s Local Authority in the delivery of operational objectives, plans and targets and to review and monitor these to improve team/directorate policy by operating effectively as a member of our client’s

To manage own time and deliver objectives and targets ensuring the development of professional practice and updating personal knowledge of local and national legislation.

To ensure that all team members work together to safeguard the well-being of all our Young People.

To complete key working sessions with allocated Young Persons in line with Key Working expectations.

ADMINSTRATIVE WORK

To update knowledge on Young Peoples’ educational, behavioural and scheduled appointments that have taken place during your absence.

Communicate effectively and consistently with team members to ensure all relevant knowledge is shared.

To record accurately and factual when communicating updates both internally and externally.

Dealing with any general enquiries and telephone enquiries professionally, ensuring messages are left for the appropriate person/s and appointments recorded.

To ensure all reports are completed by due date and all reports are factual and relevant providing a true reflection of the Young Persons current status.

Complete a brief summary on each Young Person at the end of your shift to maintain best practice.

When necessary complete reports regarding incidents/damages involving our Young Persons, ensuring company protocol is followed.

KEY WORKING

To organise and plan regular, effective key working sessions that assist in the development of individuals life skills as per Key Working Expectations policy.

To assess and ascertain the level of independency the Young Person can demonstrate through observation, questioning and workshop sessions.

To ensure workshop sessions are evidenced, recorded and signed by both parties.

To encourage, support and promote the individual to develop their own potential.

To support the Young Person with organisational needs and time management skills i.e. when shopping, visiting the dentist/doctors etc.

To support the Young Person in bedroom maintenance and management, completing their laundry whilst prompting the individual to take the lead and be as independent as possible.

To ensure the views and opinions of the young people are sought, listened to and reported back in order to help determine individual needs.

To work within our client's policies/procedures, particularly regarding Child Protection and Safeguarding policies, Health & Safety practices, Key Worker Expectations, Employee Code of Conduct, Lone Worker policy etc. 

 To arrange any meeting/appointments required and support where needed.

To always act in the Young Person’s best interest and coordinate constructive activities that support their needs.

CREATIVITY AND INNOVATION

Demonstrate a creative and reflective approach to the design and delivery of services.

Continuously demonstrate and develop better understanding of ethical issues in relation to working with Young People. (e.g. Confidentiality, Child Protection and Equal Opportunities, etc).

Work with Young People to identify barriers to participation and develop process to support into positive outcomes.

Where required, to implement and generate informed, educational workshop sessions providing practical and written evidence, ensuring best practice is shared with colleagues.

To report write, correspond with external professionals, take minutes at staff meetings etc.

To undertake training and development, as identified in work supervisions and appraisals. To build up a work development training matrix. This will be supported by the companies Managing Director.

To utilise notice boards to encourage participation of workshop sessions/external courses.

CONTACTS AND RELATIONSHIPS

Make and contribute in assessments of individual Young People’s needs and abilities.

To attend and contribute at meetings as required

To liaise with other Support Workers/Admin so that a continued, relevant support system is in place regarding your allocated Young Person/s.

Provide sustained support to Young People through regular contact, monitoring and review of progress.

To encourage close working relationships with external bodies by remaining professional at all times during contact.

DECISIONS

Whilst working independently, to apply own judgement using own initiative to make decisions should the situation require whilst following our client’s guidelines.

That adequate consequence is implemented when necessary and communicated to other team members.

To understand our clients policies and procedures ensuring you have effectively understood all legislation and Child Protection Acts.

NIGHT DUTY

Utilising time to spend with Young People

Accountable for the safekeeping, correct use and maintenance of the resources used including office phones, cleaning materials, subsidence items, manual data, and IT equipment including laptop.

Completing general cleaning and maintenance duties.

Aiding with room preparation and clearance.

 

Completing Health and Safety checks on Young Peoples rooms, ensuring clean and tidy. Working alongside Young People to ensure any tasks identified are completed.

Following company protocol when assistance is required. Ensuring all relevant bodies/professionals are informed / contacted and relevant internal forms completed.

WORK ENVIRONMENT

Work Context - Working alongside Young People there is a risk of verbal and

physical abuse/violence from the young person from which Health and Safety issues may arise. At these times company procedure must be followed and adhered too, ensuring all relevant bodies are notified and informed.

 

Working Conditions –To ensure client’s safety protocol is followed at all times by ensuring Young People are not given access to the office or its contents unless previously agreed with the Managing Director.

Work Demands - Meet agreed and imposed deadlines for reports, information / data collection and performance management. To effectively manage the workload, demonstrate good time management and a requirement to work evenings and weekends, public holidays and provide on call cover as required.

Evening and weekend work will be a requirement of the role along with on-call duties including phone support.

KNOWLEDGE AND SKILLS

Essential Qualification
- A qualification/working towards a qualification in Social Work/NVQ Level 2/3/4 in Health Care, or relevant.

Essential Experience/Knowledge/Skills


- An ability to build and sustain relationships with Young People
- Experience of working with systems for referral, identification of need etc.

- Knowledge/understanding of aims and objectives of one or more of the following areas: careers education and guidance, personal, social and health development, drugs education, work related learning and relevant legislation and policy

- Ability to analyse, interpret and identify development needs of young people and ability to devise a plan of action to meet these needs

- Administrative skills and computer literacy

- Ability to demonstrate a creative and reflective approach to the design and delivery of learning activities both in groups and one-to-one situations

- Understanding of ethical issues surrounding work with young people (e.g. boundaries of confidentiality, child protection, equal opportunities)

- Ability to communicate at a high level orally and in the written form

- Able to work as part of a team and on own using own initiative

- Organisational skills (completer/finisher)
